On 03/21/2023 at 9:30 AM Licensing Program Analyst (LPA), A. Curry and Licensing Program Manager (LPM), L. Dyson conducted an unannounced random inspection. LPA met with designated site principal, Caroline Jones . This is a preschool age program licensed for 88 children which operates Monday – Friday from 8:30 AM – 3:00 PM. Per the site principal there are 40 children enrolled.

LPA A. Curry and LPM L. Dyson began facility tour. All areas identified on the facility sketch were inspected. LPA observed 26 children with 6 staff. Teacher-child ratios were observed to be in accordance with Title 5 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times.

LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the office is used as an isolation area. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. LPA observed toxic cleaning solutions being stored in an area used for food preparation (See 809D). Staff immediately placed the cleaning solution in the food storeroom, There is drinking water available in all indoor classrooms and water cups are taken outdoors. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. When LPA arrived, the facility did not have one or more functioning carbon monoxide detectors (See 809D). The facility was able to retrieve a working carbon monoxide detector during today's visit.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition and is free of hazards. LPA inspected newly installed play equipment. Play equipment is age appropriate and secure. Areas around and/or under climbing equipment have cushioning material to absorb a fall. Based on observations today, the new play equipment is approved to used by children. Site principal states there are no weapons or firearms on the premises.

Sign in and out sheets were reviewed. Staff and children records were reviewed. Verification of CPR/First Aid certification was reviewed.



This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by ensuring faucets used for children do not exceed the acceptable amount of lead allowed in a child care facility, which poses/posed a potential health, safety or personal rights risk to persons in care. The kitchen food prep sink, room A sink, and room D sink tested higher.

By 04/21/2023 the facility will submit a written plan on how they are going to repair, permanently remove, or retest the sinks that exceeded the acceptable amount of lead allowed in a child care facility.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by ensuring toxic cleaning solutions are not being stored in areas used for food preparation and only being stored in the food storeroom, which poses/posed a potential health, safety or personal rights risk to persons in care. Although the staff immediately placed the cleaning solutions in the food storeroom, another analyst spoke to the facility about this same issue during last years annual inspection.
