On 03/12/2024 at 9:19am, Licensing Program Analysts Janai McClain and Catherine Fernandes arrived on a case management and met with Director Erica Harrell. There were 64 preschoolers in care with an additional 10 staff members.

On 03/06/2024 an incident was self reported to Community Care Licensing regarding an incident that occurred on 03/04/2024 where a child was being held by a work study student. Interviews indicated that there was a personal rights violation.

During the inspection LPAs reviewed center documentation and found that there were multiple incidents with children and staff being hurt at the facility. Although the facility is providing supervision and are within the appropriate ratio, additional support is needed to ensure that all children's needs are being met while in care.
LPAs informed Director Harrell that this report dated 03/12/2024 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the safety of children in care. Also, LPAs informed the Director to provide a copy of this licensing report dated 03/12/2024 which documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

See 809D for deficiencies cited during today's inspection.
