On 11/20/2024 at 9:30 AM, Licensing Program Analyst (LPA) Christina Watts conducted Case Management inspection at OUSD - Highland. LPA met with Lead Teacher, Harold Watkins and explained the purpose of this visit. During today's visit, there were 31 preschool children in care with 7 staff in 3 classrooms. Criminal Record Clearances are held though OUSD in agreement with Community Care Licensing.

LPA is following up on a self reported Unusual Incident. On 11/05/2024, S2 and S3 took about 3-7 preschoolers outside and wait for parent pick up. Staff interviews stated multiple parents came to pick up their child at one time. Staff interviews stated they thought C1 had been picked up by their parent. Per staff interviews, S2 and S3 came inside the classroom with 2 preschool children. S2 stated that C1's parent came to pick up C1 as they were coming inside the classroom. C1's parent was looking for C1 when S1 told S2 to go outside and see if C1 is outside. Staff interviews stated that S2 opened the door to the playground and C1 was waiting outside the door alone. Staff interviews stated C1 looked fine, as C1 was waiting outside the door. LPA was informed that a similar incident occurred around 4-6 months ago when C1 was left outside without supervision for about 1-10 minutes. California Code of Regulation, Title 22 states that facility is required to have 100% visual supervision of children in care. Also, staff interviews stated they informed their Site Supervisor of the first occurrence when the child left unsupervised however facility failed to contact licensing to report an incident. California Code of Regulations, Title 22 state thats a facility must report ALL unusual incidents to licensing within 24 hours of acknowledgement of occurrence. This facility is in violation of California Code of Regulation, Title 22.

LPA Christina Watts informed Lead Teacher Harold Watkins that this report dated 11/20/24 with 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

Furthermore, LPA Watts informed the Lead Teacher, Harold Watkins to provide a copy of this licensing report dated 11/20/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

LPA Christina Watts informed Lead Teacher, Harold Watkins that this report dated 11/20/2024 documents a Type B citation. Type B citation(s) are a potential risk(s) to the health, safety, or personal rights of children in care.

Facility had a staff meeting on 11/07/2024 regarding the incident that occured and how it will not occur again. Facility discussed protcol and a new procedure to make sure child is not left unsupervised. LPA will clear this deficiency as of 11/20/2024.

By COB 12/02/2024, Facility stated will report the first incident when child was left unsupervised to licensing. Failure to report incident by due date may result in a ciation.
