On 10/1/25, Licensing Program Analyst (LPA) Briana Plumboy met with Executive Director Beth Zygielbaum for the purpose of an UNANNOUNCED COMPLAINT INSPECTION. Present for this inspection was 28 preschool age children and 9 staff.

The allegation that the staff do not ensure that children's records are kept confidential has been SUBSTANTIATED. Based on LPAs observations and tour of the facility, the office where the staff and children's files are stored was left open on 9/15/25, and the drawers of the file cabinet where the files were stored was not secure. The preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Title 22, Division 12, Chapter 1, Section 101221(c)(1) is being cited on the attached LIC. 9099D.
A notice of site visit was given and must remain posted for 30ays.
Appeal Rights were given and discussed. An exit interview was conducted.

On 10/1/25, Licensing Program Analyst (LPA) Briana Plumboy met with Executive Director Beth Zygielbaum for the purpose of an UNANNOUNCED COMPLAINT INSPECTION. Present for this inspection was 28 preschool age children and 9 staff.

The allegation that unqualified staff are providing care and supervision to daycare children has been SUBSTANTIATED. Based on LPAs observations and record review, 1 out of 3 classrooms was operating with 2 teacher aides on 9/15/25. S5 and S10 were under the impression S5 was a fully qualified teacher, but through record review, it was found S5 needs to complete 3 more units to be a fully qualified teacher. Per S10, usually there are 2 other qualified teachers in that specific classroom, but they were both off work on 9/15/25. The preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Title 22, Division 12, Chapter 1, Section 101216.2(e) is being cited on the attached LIC. 9099D.
A notice of site visit was given and must remain posted for 30 days.

On 9/15/25, LPA Plumboy spoke with Beth Zygielbaum about the drawer with staff and children's records being accessible and not secure. Executive Director bought a locked file cabinet and is now utilizing it to safe keep children and staff records.

On 9/17/25, Executive Director Beth Zygielbaum submitted a request for an exception for one teacher aide, as well as has a plan in place for maintaining ratio with teachers and staff.
