On December 4th, 2024 at approximately 3:05PM, Licensing Program Analyst (LPA) April Wright arrived at for an unannounced Annual/Random inspection, and met with School age Site director Payton McQuillan. Present today were thirty-five (35) school age children and two (2) fingerprint cleared staff personnel present during the inspection. The facility is in ratio today. The facility was toured for a health and safety inspection. Hours of operation are Monday through Friday, 7:00am to 6:00pm.

CLASSROOM: Facility is a TK-5th grade program which offers before and after school care for children. Facility is located in a bungalow in the rear of the Durham Elementary School campus. There are adequate play and learning materials available. The floors, furniture, and equipment are age appropriate and in good repair. There is adequate heating/air conditioning, ventilation and lighting for safety and comfort. Drinking water is available inside and outside of the center. LPA observed and Site Director confirmed that are no toxins, medicines, cleaning products or hazardous materials visibly present during today's inspection and were made inaccessible to children in care. There is proper individual storage space for each child, hooks for backpacks are located on the wall upon entry to the room. The isolation area for sick children is near director's desk in a chair or mat, away from other children in care. The center has smoke/carbon monoxide detector working telephone, and two (2) fully charged 3A40BC fire extinguishers located in the classroom. LPA observed and Site Director confirmed that there are no pets, firearms or weapons located in the facility. LPA observed and Site Director confirmed that there no pools, hot tubs, or any bodies of water present at the facility. At least one person trained in CPR/First aid is present at the facility when children are present.
BATHROOMS: Bathrooms for children are at located across the yard of Durham Elementary school campus. Staff bathrooms are separate from the children's bathrooms and are also located at in rear of Durham front office. All bathrooms for staff and children are neat and in sanitary condition. All sinks and faucets are in safe and sanitary operating condition and children can reach sinks/faucets. Supplies are

FOOD SERVICE AREAS: This facility provides snacks only to children in care. There are weekly snack menus posted at the facility. LPA inspected all storage containers used for solid waste. LPA observed all containers have tight fitting covers that are in good condition and repair. Facility has a refrigerator which was inspected by the LPA and is free of any evidence of rodents, vermin or insects.

OUTDOOR PLAY AREA: Facility shares the play yard with Durham Elementary school and has a schedule in place for use. There is a play structure with slide that has wood chip cushioning to absorb falls and that is anchored for stability.

RECORDS: All individuals subject to criminal record review have a clearance or exemption and have been associated to the facility. Six (6) children's files and three (3) staff files were reviewed including the directors file. All staff files have required health screening and Employee Rights and all children files contain Identification & Emergency, Personal Rights, and Medical Consent forms. LPA reviewed the staff and children's roster and obtained a copy. Mandated Reporter Training certificates and CPR/First Aid certificates are current and up to date for all staff members staff. The center is in compliance with sign in and out procedure LPA viewed sign in/out sheets for accuracy. Disaster drills are being conducted monthly and the last one conducted was on 11/20/2024. All required documents are posted in a public accessible area.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Site Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives
outlined in PIN 21-21.1-CCP. PIN 22-05-CCP.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.



A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Site director Payton McQuillan.
