On September 12, 2024 at approximately 12:20 PM, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux conducted an unannounced 1 Year Required Inspection and met with Director, Karen Berry and Co-Director Martha Bali. LPA disclosed the purpose of the inspection and was granted entry into the facility. There were thirty- five (35) children present during this inspection and ten (10) staff members (includes administrative staff). The facility was toured for a health and safety inspection. The hours of operation are 7:00 AM - 6:00 PM, Monday - Friday. Facility is operating within capacity limits and ratios at time of LPA's visit.

CLASSROOMS: There are five (5) classrooms being used and were inspected. During today's inspection Room 5 is not being used, storage area. The classrooms were inspected and there were play and learning materials available for the children. The floors, furniture, and equipment are age appropriate and in good repair. There is adequate heating/air conditioning, ventilation and lighting. Families supply water bottles for the children it is refilled with filtered water as needed. There is proper individual storage space for each child. The isolation area for sick children is in the director's office, away from other children in care. The facility has wired smoke detector, a carbon monoxide detectors in each classroom (tested and functioning) , working telephone, pull down fire alarm system, and six (6) fully charged 3A40BC fire extinguishers through out the facility. Children are signed in and out of facility through an electronic app system called ProCare..

FOOD SERVICE AREAS: The facility does not provide lunch, snacks are provided in the AM and PM snacks for the children. There are monthly menus posted at the facility. LPA observed an area where they have a refrigerator this area is clean and free of evidence of rodents. Food is protected from contamination and, LPA reminded staff to discard contaminated/expired food immediately. Low-fat 1% milk is being served at this facility and pesticides and other similar toxic substances are not stored in cabinets that contains food. All storage containers for solid waste have tight fitting covers that are in good repair.

BATHROOMS AND TOILETING AREAS: The staff's bathroom is separate from the children's bathroom. All sinks and faucets are in safe and sanitary operating condition. The children can reach the sinks and toilets. Supplies are available to the children. There is a diaper changing table in Room 1 that is in arm's reach of the sinks.

OUTDOOR PLAY AREAS: The playgrounds at the center for the children to play is fully fenced. The playground has large trees for shade. Staff will bring out a water jug with disposal cups to keep the children hydrated. There's a play structure, which is anchored that has cushioning to absorb falls. There is a shed located in the off limit area that is locked and blocked by a gate.

RECORDS: All individuals subject to criminal record review have a clearance or exemption and have been associated to the facility. LPA requested and reviewed ten (10) children's files and seven (7) staff files were reviewed around 2:10 PM. All staff files have required Health Screening, Employee Rights, and Criminal Record Statements and all children files contain Identification & Emergency, Parent Rights and Medical Consent forms. LPA reviewed the facility roster and obtained a copy. At least one opening/ closing staff member has a current CPR & First Aid Certificate. Mandated Reporter Training was discussed, and certificates were reviewed. The facility representative's CPR and First Aid certificate is current. The center is in compliance with the sign in and out procedure. Disaster drills are being conducted at least once every 6 months and the last one conducted was on 07/25/2024. All required documents are posted in a public accessible area.

HEALTH RELATED SERVICES: IMS is being provided at this facility. LPA inspected the medication, which is stored in a safe place that is inaccessible to children in care. Each of the medications has an unaltered label with the child’s name and date of issuance. A refrigerator shall be used to store any medications that requires refrigeration. The center is equipped with fully stocked first aid kits that is available in the classroom.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

California Law requires Child Care Centers licensees to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. Roster of the children must be properly maintained, and fire/disaster drill every six months must be documented.

Facility was informed that as of September 1, 2016, a person may not be employed or volunteer at a child care facility unless he or she has been immunized against influenza, pertussis, and measles or qualifies for an exemption pursuant to Health and Safety code 1596.7995 and 1597.662.

Lead Testing: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

MyChildCarePlan.org: Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


No deficiencies were cited today. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Exit interview conducted and report was reviewed with the Director, Karen Berry and Co-Director, Martha Bali.
