On 12/11/2025 at 1:00PM Licensing Program Analyst's (LPA's) Kareeca "Reeca" Sykes and Mone Flores conducted an Unannounced Subsequent Complaint Investigation at Lakeshore Children's Center. LPA's met with Authorized Facility Representative Michelle Leprohn and explained purpose of the investigation. During the visit LPA's did not observe any school age children in care, although the facility representative informed LPA's there are currently 45 school age children enrolled. Finding for the above allegation was delivered during the inspection. Complainant alleges that there was an Absence of Supervision at the facility. During the course of the investigation, LPA's inspected the facility, reviewed records and conducted interviews. It was determined that there have been multiple occasions where a lack of adequate supervision at the facility has resulted in conflicts and physical altercations between children. Based on the interviews and all information obtained throughout the investigation, the preponderance of evidence standard has been met. Therefore, the allegation is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 is being cited on 9099-D page.
Exit interview was conducted with Authorized Facility Representative Michelle LeProhn. Appeal rights were provided. A NOTICE OF SITE VISIT WAS ISSUED AND MUST BE POSTED FOR 30 CONSECUTIVE DAYS.

By 12/12/2025 Close of Business, the facility will submit a written statement to LPA, explaining the importance of 100% supervision and how the facility will remain in compliance.

Based on interviews, the facility did not comply with the section cited above when C1 and C2 were left unsupervised in the "ladies dressing room" area off the green room on multiple occasion which resulted in a physical altercation between the two children which poses an immediate risk to the health, safety or personal rights of children in care.

Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
