On 11/01/2022 approximately at 8:45AM Licensing Program Analyst (LPA) Kelly Phan met with Darlene Johnston (Center Director) for an unannounced Required 1-Year Inspection. Present for this inspection are 54 preschool aged children along with 9 fingerprinted and associated staff members. Facility operating hours are 7AM-5:45PM Monday to Friday. The facility was toured to conduct a health and safety inspection.

At 9:20AM, LPA and director toured the outdoor play area, which is fully gated. All play equipment was observed to be operable and age-appropriate during this inspection. Areas around slides and high climbing structures have grass and wood chips to absorb falls. The facility does have a sand area for playing, which is inspected daily by the staff and has a device to prevent any cats or other animals from accessing the area. There are plenty of trees and umbrellas to provide shade. There were also several sheds that are used for toy storage that is locked. At 9:45AM, LPA toured the facility with director of all five classrooms. All classrooms had age appropriate equipment and toys. The classrooms are tidy and clean with heating and ventilation for safety and comfort. All hazardous materials and toxins including disinfectants and cleaning solutions were observed to be made inaccessible to children during today's inspection. Each classroom had individualized storage space for each child that they are able to store their personal blankets. Per director, blankets and beddings are given and cleaned weekly by the child's parents, and sleeping mats are also cleaned weekly by facility staff. In one classroom, LPA was made aware that there is a small fish tank, which has no fishes inside. LPA advised director that the fish tank needs to be fully supervised and ensure that children are not playing inside or around the area; Director understood. The facility is utilizing disposable water cups and water fountains throughout the premises for drinking water; LPA also received lead testing results completed as of March 3, 2022 as the facility is using water fountains and other water sources for food preparation. There are two separate bathrooms located in the common play area; each bathroom area has plenty of paper towels, soap, and working toilets.

At 11:40AM, 6 children files were reviewed and found to be complete; 5 staff files were also reviewed for completion and have required immunizations on file. All staff who were present at the facility has current CPR and First aid along with mandated reporter training completed. LPA also observed that the facility has a waiver to use an electronic system as their sign in/out procedure; LPA observed 54 children checked in out of 54 children present in the facility during today's inspection. The facility has several 3A40BC fire extinguishers, working smoke detector, working carbon monoxide detector, and working telephone throughout the facility, which is last checked by Coast Fire and G & S Cal Fire on 06/03/22 and 06/28/22. Earthquake drill was conducted on 10/20/22 and fire drill was last conducted on 10/25/2022. All required forms are posted in public view, including menus posted throughout the classrooms. Per director, the menu is updated monthly as the facility provides breakfast, lunch, AM, and PM snacks. LPA also toured the kitchen and storage area that is OFF limits to children; LPA also reviewed children's medications are also stored in the kitchen area, which has proper documentations and are in original unexpired containers. LPA did advised director that original containers cannot have any other labels that prevent staff from reading the instructions provided for each prescribed and non-prescription medications. Director understood. The facility also has a separate area to store first aid supplies away from children as well as a separate bathroom and eating area. Facility roster was reviewed and a copy was obtained.

There were no deficiencies cited during today's inspection.

Appeal rights and a notice of site visit notice was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the facility director, Darlene Johnston.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. This facility provides IMS to children in care. The facility is following and have developed IMS plan on file. When any changes to the IMS plan is made, an updated Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Licensee was reminded that California Law requires licensed Child Care Centers to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. LPA informed the Facility Representative that all forms can be downloaded at www.ccld.ca.gov and encouraged the Facility Representative to email childcareadvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. Licensee was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com.
