On February 7th, 2025 at approximately 9:25am, Licensing Program Analyst (LPA) April Wright arrived for an unannounced Annual Random inspection and met with Site director Nadia Amiryar. Program is located in a portable in the rear of the Joseph Azevada Elementary school campus. Present during inspection were four (4) school age children. There were no additional staff personnel present at time of inspection. The facility is in ratio today. The facility was toured for a health and safety inspection. Hours of operation are Monday through Friday, 7:00am to 6:00pm.

CLASSROOM: School-age program is for children TK - 5th Grade. There are adequate play and learning materials available which are age appropriate in each classroom. LPA visually inspected the floors, furniture, and equipment which were observed to be in good repair, free of hazards or damage. There is adequate heating/air conditioning, ventilation and lighting for safety and comfort. Drinking water is available inside and outside of the center. There is proper individual storage space for each child. The isolation area for sick children is the playhouse area which is away from other children in care. The center has working smoke/carbon monoxide detectors, working telephone, and a fully charged 2A10BC fire extinguisher. LPA observed and Site Director confirmed that there are no pets, firearms or weapons located in the facility. LPA observed and Site Director confirmed that are no toxins, medicines, cleaning products or hazardous materials visible during today's inspection and were made inaccessible to children in care. At least one person trained in CPR/First aid is present at the facility when children are in care.

BATHROOMS: Children's bathrooms were inspected by LPA to be in sanitary condition. The staff's bathroom is separate from the children's bathrooms. Children's restrooms are located directly across from the portable. All sinks and faucets are in safe and sanitary operating condition. Children are able to reach the sinks and toilets without difficulty.

FOOD SERVICE: This facility provides snacks only to children in care. Center has a kitchen prep area that is neat, orderly and off limits to children in care. Facility has a refrigerator which was neat and free of evidence of rodents, vermin or insects as observed by the LPA. Sink area in the classroom was observed by the LPA to be neat and in sanitary condition. LPA did not visually observe any chemicals or cleaning agents in the kitchen prep area during the inspection. There are monthly snack menus posted at the facility on the parent board upon entry. All storage containers for solid waste have tight fitting covers that are in good repair.

OUTDOOR PLAY AREAS: Center shares the play yard with Joseph Azevada Elementary school where a schedule of usage is in place. There are 2 play yards for the centers use which have play structures with slides with wood chips for cushioning to absorb falls and that is anchored for stability. LPA observed no visual signs of damage, hazards or defects. LPA observed and Center Director confirmed there are no pools, hot tubs, or any bodies of water present at the facility.

RECORDS: All individuals subject to criminal record review have a clearance or exemption and have been associated to the facility. Four (4) children's files and three (3) staff files including directors file were reviewed. All staff files have required health screening and Employee Rights and all children files contain Identification & Emergency, Personal Rights, and Medical Consent forms. LPA reviewed the facility roster and obtained a copy. Mandated Reporter Training and CPR/First Aid certificates were reviewed and are current. The center is in compliance with the sign in and out procedure. Disaster drills are being conducted every 6 months and the last one conducted was on 1/30/2025. All required documents are posted in a public accessible area.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


A notice of site visit was given and must remain posted for 30 days. Report was read and reviewed with Site Director Nadia Amiryar.
