On 02/14/2023. Licensing Program Analyst (LPA) Ashley Curry conducted an unannounced Case Management inspection at OUSD Howard School. LPA met with head teacher Lottie Lynch, and explained the purpose of today's inspection. The facility self reported an unusual incident where a child touched another child inappropriately. There was a lack of supervision that lead to inappropriately touching between children on multiple occasions, which is an immediate risk to the health and safety of the child. Type A deficiency is being cited for lack of supervision. LPA toured the facility, made observations, and conducted interviews.

LPA A. Curry informed facility representative Lottie Lynch this report dated 02/14/2023 document(s) 1 type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA A. Curry informed the facility representative to provide a copy of this licensing report dated 02/14/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted, appeal rights were given, and report was reviewed with head teacher Lottie Lynch.

The facility immediately moved the children beds away from each other during nap time.

By 02/28/2023 the facility will priovide written plan on how to ensure staff will have visual supervision of children at all times.
