On 6/22/2022 Licensing Program Analyst (LPA) Diana Campos arrived at the facility and met with Center Director Roselyn Ly for an Unannounced Annual Required Inspection. Present during this inspection were 5 staff and 16 preschool children in care. There is one large classroom. Sign-in/sign-out sheets list the time and parents' full signature. There is a working phone. The last fire/emergency drill was 5/17/2022. Proper posting requirements were met including Waiver. There is a carbon monoxide detector, fire extinguisher, and emergency earthquake supplies. Classroom furniture is in good condition. There are toys, books, crafts and educational supplies available. Floors were clean and safe. There is adequate heating, ventilation, and lighting. Bathroom toilets and sinks are working properly. There were adequate bathroom supplies. Kitchen area (including storage areas for food) was clean. No insects or pests were seen. There were storage containers with lids for solid waste. A sample of food product was examined for freshness and expiration dates; there was adequate food for emergencies. Food is delivered from the Oakland Head Start Central Kitchen. The center supplies breakfast, lunch and snacks. Menus were posted. There is sufficient storage area for each child's belongings with cubbies. There is adequate napping equipment (mats). Bedding is sent out to be laundered. There is drinking water available at all times inside the center through a fountain; and outside with pitchers and paper cups. Director stated that there were no bodies of water or firearms on the premises. First aid kits are located in the classroom. Cleaning supplies are inaccessible to children, placed in a locked cabinet as well as in a locked janitorial closet. Outside play area is securely fenced and is safe and free of hazards. The area around the climbing structure is cushioned with material to absorb a fall.

A sample of children's files were reviewed and found to be complete. Sample reviewed lists emergency information, a medical assessment, and an Admission Agreement. IMS plans are in file. Medications are stored in a cabinet inaccessible to children. 5 Staff records were reviewed. Staff records contain a health screening assessment, immunization records, and Mandated Reporter Training certificates. The director stated that at least one person trained in CPR/First Aid is always present. All individuals subject to a criminal record review have obtained clearance or a criminal record exemption.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

There are no deficiencies cited today.

A notice of site visit was given and must remain posted for 30 days.
