On December 11, 2025, at 8:00am, Licensing Program Analyst (LPA) Randy Miranda met with Interim Director Rachita Bhandari to deliver the findings from a complaint investigation for the above allegation. LPA toured the facility and obtained a copy of the sign-in sheets and facility roster. Present for the inspection were the Interim center director, enrollment coordinator, three (3) teachers, and 10 infants in care.

Based on information obtained through interviews, record reviews, and observations, the infant center operated over ratio. The preponderance of evidence standard has been met. The allegation that the daycare was over of ratio is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Subchapter 02, Section 101416.5(b) is cited on the attached LIC 9099D.

A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Exit interview
conducted and report was reviewed with Interim Center Director, Rachita Bhandari.

Based on interviews conducted, observations, and record review, the allegation Daycare does not have enough cribs for children may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the allegation is UNSUBSTANTIATED.

Director is to draft a plan on how facility will ensure they remain within ratio. Provide training and assign permanent teachers to the infant center. Plan will be submitted to LPA no later than 12/30/2025, via email.

This requirement was not met based on observations and interviews, it was determined by LPA that the facility was out of ratio. This poses a immediate risk to the health and safety of the children in care.

LPA recommendations and requirements are the following:
1. Conduct individual staff training and create roster of attendance. Roster to be emailed to LPA once training has concluded.
2. Informative video to learn more about putting Licensing requirements into practice, Infant to Teacher Ratios in Child Care Centers
