25 | On May 21, 2025, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux met with Director, Lindsay Anderson for an unannounced case management -incident in response to an Unusual Incident that the Facility self reported to the Department on 2/21/2025 and 3/06/2025.
LPA Breaux conducted interviews with the director and staff and obtained relevant documents. Based on interviews and record reviews, the facility acknowledged that on 02/21/2025 and 3/6/2025, child(ren) were left unsupervised during transitioning from outside to inside. On 2/21/2025 C1, while transitioning from the play yard into the classroom C1 ran past S1 and S1 did not know where C1 went, C1 was found in Preschool C, per director is was about one (1) minute the child was left alone. On 3/06/2025 at approximately 4:50PM while transitioning from outside to inside, S3 closed the door and S2 heard the door jiggling, when S2 opened the door C3 was left outside unsupervised. Per director the child was outside for less then ten (10) seconds.
Absence of supervision is a Zero Tolerance violation. As a result, a Type A deficiency is issued during today’s inspection. California Code of Regulations, Title 22, Division & Chapter Number 101229(a)(1)} is being cited on the attached LIC 809D.
As a result of the Type A deficiency cited during today’s inspection, a copy of this report and LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to parents/guardians of current enrolled children in care and all children newly enrolled following a 12-month period of this report. The LIC 9224 must be signed within the next business day the children are in care and is to be kept in the children’s files.
An immediate civil penalty of $250 for repeat violation is assessed today.
A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Lindsay Anderson.
