On September 12, 2024 8:20am Licensing Program Analyst (LPA) Randy Miranda arrived to conduct an unannounced site inspection for health and safety. LPA met with Teacher Anjum but was not available to assist LPA due to maintaining ratio. After 10-12 minutes of LPA’s arrival, Teacher Sindu arrived and assisted LPA with the inspection process. Teacher Sindu and LPA toured all areas licensed for use by children in care. Also present at the time of this inspection were 1 teaching staff, 1 teacher aide, and 15 children in care (all preschool age). The hours of operation are Monday – Friday 7:30am to 6:30pm.

The facility is a single-story building at the end of a small row of shops. There are two license components operating at this location and they share the playground area. The playground is surrounded by a 6-foot privacy fence, secured with gates with inside latches too high for children to reach. All play equipment is in safe condition and free from sharp, loose or pointed parts and the areas around or under high climbing equipment has appropriate cushioned material that absorbs a fall. Shade was available in the play areas and teachers are always present. LPA observed artificial turf covering a curb, has become worn and torn presenting a tripping hazard, a Technical Violation was issued. Water testing or Lead content was completed, there are no lead exceedances found. Children bring their own bottled water from home.

The preschool consists of 3 classrooms: Little Fish, Lions, and Stars. They have a shared bathroom with 5 working toilets and sinks, and an off-limits gated kitchen. The kitchen contains a refrigerator, stove, dishwasher and area for dry goods storage. There were no sharp knives observed in the kitchen today. All food was checked and not expired, milk in the refrigerator was 1% fat. Cleaning chemicals and other cleaners are stored out of reach of children.

Sinks have functioning cold water taps with appropriate hand washing signs and sufficient amounts of towels and soap. All toilets, hand washing and cleaning areas are in safe and sanitary operating condition. Classrooms have trash cans with tight fitting lids for the disposal of solid waste.
The facility is clean and well organized with age-appropriate furnishings and equipment in good repair. Surfaces including floors and counter tops are clean and toxic free. There are ample age-appropriate toys that appear to be safe and in good condition. There are no bodies of water accessible to children in care. There were no hazardous items/toxins observed to be accessible to children in care today.

The sign in/out sheets are electronic (Brightwheel app) and were reviewed. In addition to electronic sign in, children are checked in manually, written into a notebook by the teachers. Children present were accounted for on the school attendance log.

There are two fully charged 2A10BC fire extinguishers last serviced on April 17, 2024. One in the kitchen prep area and one in Lions room. The rooms have working smoke detectors and carbon monoxide detectors in the ceilings. There is one restroom for staff, separate from the children's restrooms. Heating and ventilation is acceptable.

Disaster drills are conducted once per month, the last drill was conducted August 19, 2024. All required posted documents were found to be in compliance and prominently posted in public places.



The center currently does not administer medication. Incidental Medical Services Plan is not implemented at this time. The children's napping areas are clean and organized with sleeping pads and cubbies for the children's personal items.

Staff files were reviewed. All files were complete and in good order. Center Director CPR/1st Aid expires 10/28/2025 and Mandated Reporter certificate completed on 9/15/2023.

Children's records were reviewed: LPA requested and reviewed child roster and took a copy. All children’s records were complete and in good order except for one record. An LIC627B (Children’s Resdidential Facilities) was filled out instead of LIC627A (Child Care Centers or Family Child Care Homes). A Technical Advisory was issued.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Center Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

There was one deficiency issued during today’s inspection:

*Artificial turf in the play yard has become torn and presents a tripping hazard (Type B violation).



See 809D for deficiency cited today. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.


There was one Technical Advisory issued during today’s inspection:

*LIC627B for Children’s Residential Facilities was signed instead of LIC627A (Child Care Centers or Family Child Care Homes).
