On July 28, 2022 at 10:45 AM Licensing Program Analyst Elimika Woods met today with Site Director, Maria Soares to deliver the findings to the above complaint allegation. During the last analyst visit, interviews and a tour of the facility were conducted and a copy of the facility roster was received. On June 7, 2022 while on the playground a child escape off the premises through two different opening, A fence on the playground on the center side and at the bottom of the fence on a neighbors side fence. During the investigation LPA Woods was taken to the spot by the director were the child escaped. Based on LPA Woods interviews, police incidents reports, record reviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is to be SUBSTANTIATED.
See 9099-D A copy of the LIC 9224 has been given and explained. A copy of this report is to be given to each child's parent by the next business day and a copy of the LIC 9224 acknowledgement of receipt of licensing reports is to be signed by the parent and put in their child's file. A copy is also to be posted at the facility for 30 days. Any newly enrolled student's parents are to receive a copy of this report for one year.

The director was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. Notice of Site visit was provided and must be posted for thirty days. Exit interview conducted with director, Maria Soares.

The Site Director will submit a written plan of correction with methods and procedures to prevent structures that are in need of repair (fences) that are damage to LPA by 08/11/2022 due date via email or mail.

Based on the LPA obersavations and staff interviews, the condition of the facility indicates that the staff failed to prevent a damage fence from being used to escape.
This requirement was not met as evidence. This poses an immediate risk to the Health, Safety and personal rights of the children in care.

The Site Director shall contract a handyman or contractor to repair the facility fence and take measures to prevent escapes. The Site Director shall conduct and document a staff meeting and or training on the facility safe, clean, safe, sanitary and in good repair at all times to ensure the safety.. The Assistant Director shall forward a copies to LPA.
