25 | On 9/7/2021 at 4:20pm, Licensing Program Analysts (LPAs) Jaylena Miller and Briana Plumboy arrived at Celebration Learning Center for the purpose of a plan of correction inspection. On 8/12/21, LPA Miller cited the facility a type A violation for not complying with Alameda County guidelines by wearing mask indoors to prevent the spread of COVID-19 during the pandemic. On 8/27/21, LPAs Miller and Plumboy conducted a plan of correction inspection and as a result, recited the facility and assessed a civil penalty for the repeat violation that was originally cited on 8/12/2021.
On 9/7/21 at 4:22pm, LPAs inspected the school age program classroom #4 (Facility #013415194) first. Present for the school age were 30 children and 5 staff and three out of the thirty children were wearing mask indoors. At 4:28pm, LPAs inspected, observed, and conducted a physical census of the preschool classroom (Facility #013415193). Present for the inspection were 9 preschool children and 2 staff all who were not wearing mask and as a result, a civil penalty of $100 per day will be enforced starting 8/31/2021 until the facility comes into compliance.

After numerous attempts to bring the facility into compliance the facility still chooses to not wear mask indoors. Community Care Licensing (CCL) will require the facility to participate in a non-compliance conference with the Oakland Southeast Regional office on 9/14/2021 at a time determined by CCL.
Appeal rights left at the center at the office door. A notice of site visit was left at center at the office door and must remain posted for 30 days, exit interview not conducted due to refusal from facility director. |
