On March 24, 2023 at approximately 8:45am, License Program Analyst (LPA) Russ Haderer arrived unannounced to conduct an inspection for health and safety. LPA met with the site supervisor, Teresa Saunders. Present upon arrival was 2 teachers and no children in care. The hours of operation are Monday to Friday 7AM to 6PM.

The facility is located in a portable building (F01-F01A) located in the back of Roosevelt Elementary nearest Dutton Avenue. The portable has two separate classrooms; a small kitchenette with two sinks with a microwave and toaster over; and a staff office. There are separate boys and girls bathrooms with two toilets and a sink in each one and a separate all-gender bathroom for staff. There are cubbies available for all children's personal possessions. All hazardous materials and toxins are inaccessible to children in care.

The outdoor play area is shared with the elementary school. A waiver is in place to ensure a schedule is maintained to prevent the children from co-mingling age groups and children are supervised by facility teaching staff to ensure this. All play equipment is in safe condition and free from sharp, loose or pointed parts and the areas around or under high climbing equipment has appropriate cushioned material that absorbs a fall. There are no bodies of water accessible to children in care.

All required posted documents were present. Disaster/fire drills are conducted monthly by the main school, the last drill was completed on 3/16/2023. All toilets, handwashing and cleaning areas are in safe and sanitary operating condition. All surfaces accessible to children are clean and toxic free. The sign in/out sheets were reviewed and complete. The snack menu is posted and visible for review. All food/snacks were within expiration date.

Lead testing for consumable water was been completed 2 weeks ago. Results to be publicly posted within 180 days of test results.

The facility is open in the morning for children before they attend their regular schooling and also open in the afternoon for an afterschool program. The facility is clean and well organized with age appropriate furnishings and equipment in good repair. Surfaces including floors and counter tops are free of hazards and toxins. The smaller classroom has a sink with a water fountain for the children. There are solid waste trash cans in both rooms with tight fitting lids and cubbies for children to store their belongings. Both classrooms have first aid kits hanging in backpacks on the wall near the exit door. The facility has fully charged 3A40BC fire extinguishers last serviced October 27, 2021. Smoke alarm system is built into the classroom and managed by the school district. LPA did not observe any carbon monoxide detectors in the classrooms and was advised these detectors are built into the buildings fire alarm system. Hot water is not functioning, a work order has been placed to building maintenance to make the correction. A technical violation was issued.

Children’s records were reviewed. A facility roster was requested and a copy made for the LPA. All files selected were complete and well organized. No children currently enrolled require any IMS service.



Staff records were reviewed. A personnel roster was requested and a copy made for the LPA. One staff member has not completed the Mandated Reporter training, the balance of all files selected were complete and well organized. The staff member signed the forms acknowledging to report child abuse so a technical violation was issued with the stipulation that mandated reporter training must be completed (child care provider AB1207) and renewed every two years.LPA reminded site director of the following: Mandated Reporter training and CPR/First Aid is also renewed every two years.

Site Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Two technical violations were issued today, one for hot water and one for a missing mandated reporter certificate. No other deficiencies were found or cited at facility during this inspection.

A notice of site visit was given and must remain posted for 30 days.
