On 12/11/2025 at 9:00am, Licensing Program Analyst (LPA) Catherine Fernandes met with Director Chantel Pratt for an Required Annual Inspection. The center was toured for a health and safety inspection. The facility operates from Monday through Friday from 8:00am–6:00pm. Present in care were 66 preschool age children and seven toddlers with an additional 18 staff members. The teacher- child ratio is within compliance according to regulations. The center operates out of a commercial property and consists of six classrooms and enclosed two outdoor play areas. The center has a school age component in one classroom (located on the far right side of the mezzanine) license number 013415929 which is inactive at the moment. The center provides a variety of daily activities which includes quiet and active play, rest and relaxation, eating and toileting.

Classrooms: The toddler classroom is the first classroom on the left side of mezzanine which is called MMZ (movement matter zone). The room floors and surfaces were observed to clean and safe. Furniture and equipment were maintained in good condition, free of sharp, loose or pointed parts. The heating, lighting and ventilation is adequate. The activities and toys appear to be age appropriate for the children in care. The children have their own space to store personal belongings. During today's inspection, LPA did not observe cleaning supplies, or toxic items accessible to children. There a working telephone number for the center. Director has confirmed fixtures, furniture and equipment in the center has not been banned or recalled.

Bathrooms: There is a total of 11 toilets and 18 sinks available for the children which are sanitary and in operational condition. There is a changing table in the toddler classroom that is next to a sink. There are three staff bathrooms, two are located in the entryway of the center, and one upstairs in the staff lounge.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Outdoor yard: The toddler yard is located on the left side of the building. The outdoor activity space is maintained in a safe condition, and free of hazards. The play area is fully fenced with shaded areas for the children. There is a play structure that is bolted to the ground. The climbing equipment is cushioned to absorb a fall. There are no pools, fishponds, fixed in place wading pools or similar bodies of water on the property.
Toddler Sleeping: Staff confirmed toddlers are not being swaddled while napping. The center is utilizing electronic sleep check which includes the date, the infants name, the time if each 15 minutes check and the initials of the person conducting the check.
Food Service: The kitchen area was maintained in a clean manner. The center prepares and provides snacks to the children while in care and lunches are brought from an outside vendor. All the children have personal water bottles and access to clean drinking water while indoors and outdoors. The center has conducted their lead testing April 2022 and does not have lead exceedances in the sinks used for food prepping or drinking. Cleaning equipment was observed to be stored in separate areas from the food supplies and were inaccessible to children. There is a tight fitting lid on the trash cans that contained food.
Fire System: The center has maintained their fire clearance and the fire system is hardwired fire department. LPA tested the carbon monoxide detector in the one of the classrooms, there is fully charged fire extinguishers in the center and pull down fire alarms at each exits of the building. The center has a first aid kit in every classroom and conducts and electronically documents their disaster drills every six months with the last one done on 11/4/25.
Postings: All required forms are posted in a visible area for parents to see.

Records: There is an electronic and written sign-in and out log sheet for the children in care which includes the person signing the child in, the time and date.
A random sampling of five children's files were reviewed. LPA reminded the center that all toddlers need to have consent to participate in the toddler option. LPA reviewed four staff files and observed teacher qualifications and current mandated reporter certificates. The center has provided proof of at least one staff member with a current CPR certificate.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

CCC Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.

LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with Director Pratt and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed the Director of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
