On August 28, 2025, at 8:00am, Licensing Program Analyst (LPA) Randy Miranda arrived unannounced for an annual inspection for compliance to Health and Safety. LPA met with center director, Elizabeth Villa
and toured all areas licensed for use by children in care. Present during the inspection were 13 adults (11 teachers and 2 administrative staff) and 47 children in care (34 preschool age and 13 toddler age). The facility is within ratio and capacity. The hours of operation are Monday - Friday 7:00am – 6:00pm, Summer hours are Monday - Friday 7:30am - 5:00pm.

The facility is located on church property, situated at the back of the campus on the south side. It consists of four classrooms and a private playground area for the children, also located on the south side. The playground is enclosed by a six-foot privacy fence and secured with gates featuring interior latches positioned too high for children to reach. All play equipment is in safe condition, free from sharp, loose, or pointed parts. Areas around or beneath high climbing structures are equipped with appropriate cushioned material designed to absorb falls. Shade is available throughout the play areas, and teachers are always present to supervise. Children bring bottled water from home, and additional drinking water is provided by staff using a water cart available both indoors and outdoors.

Each classroom is equipped with a fully charged 3A40BC fire extinguisher, and the front lobby contains a fully charged 2A10BC fire extinguisher. The most recent fire extinguisher annual inspection was conducted on October 11, 2024, by Coast Fire Equipment. Functional carbon monoxide detectors and fire alarms are installed on the high ceilings and have been tested to ensure proper operation. Heating and ventilation systems are in acceptable condition. Disaster drills are conducted at least once every six months, with a fire drill held on June 2, 2025, and an earthquake drill on April 30, 2025.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The facility is clean and well-organized, with age-appropriate furnishings and equipment in good repair. Surfaces, including floors and countertops, are clean and free of toxins. There is a generous supply of age-appropriate toys that appear safe and in good condition. No bodies of water are accessible to children in care, and no hazardous items or toxins were observed to be within children's reach during today's inspection.

The kitchen area is designated as off-limits and contains a refrigerator, washer/dryer units, a purified water dispenser, a sink with hot and cold running water, a covered trash bin, and a recycling bin. Currently, the school does not provide lunch or primary snacks; all food is supplied by parents. However, the school maintains an adequate supply of alternate snacks for use if needed.

The children's napping areas are clean and well-organized, equipped with sleeping pads and cubbies for storing personal items. All required documents were properly posted. A waiver for scheduling outdoor playtime is in place for children in the play area, and this waiver is also visibly posted.

There are three children's bathrooms and a separate staff bathroom. All sinks have functioning hot and cold-water taps, accompanied by appropriate handwashing signage. Trash cans with tight fitting lids are available, along with sufficient supplies of towels, soap, and diapers. All toilets, handwashing stations, and cleaning areas are in safe and sanitary operating condition. Classrooms are also equipped with trash cans featuring tight-fitting lids for the disposal of solid waste. Manual sign-in and sign-out sheets are maintained and verified by administrative staff.

Children’s records were reviewed, and all selected files were found to be complete and up to date. Staff files were also examined, confirming that all individuals required to undergo criminal background checks have been cleared and are appropriately associated with the facility. At least one opening or closing staff member holds a current Pediatric CPR/First Aid certification. The director was reminded of their responsibility to renew CPR/First Aid and Mandated Reporter certificates every two years.



All documents required to be posted were appropriately posted on the walls: License; Emergency Disaster Plan; Earthquake Preparedness checklist; Notification of Parents Rights; Personal Rights; Child seatbelt laws; menus; and daily activity schedules.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Site supervisor was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Lead testing is complete and documented.

Effective August 1, 2003, California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child’s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days. Site Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
