Licensing Program Analyst (LPA) Kassandra Medrano conducted an unannounced inspection in order to deliver findings on the complaint investigation for the above allegation. LPA Medrano met with the Director, Soni Singh to discuss complaint allegation and finding.

It was alleged that staff yell at children while in care. LPA Medrano interviewed staff, management, and parents. Based on interviews, observations, as well as information gathered; it was found that staff speak inappropriately to children in care.Therefore violating the children's personal rights. The allegation noted above is thus found to be SUBSTANTIATED, meaning the allegation is valid because the preponderance of the evidence standard has been met.

A Type “A” violation was issued today. The center was advised to provide a copy of the Evaluation Report and the Type “A” Deficiency cited to the parents and guardians of children currently enrolled in care and to parents of newly enrolled children during the next 12 months. A signed and dated LIC 9224 shall be maintained in all Children's files. This report and appeal rights were provided and reviewed with the Director, Soni. Notice of Site Visit and Type A Citation shall remain posted for 30 days.

Licensing Program Analyst (LPA) Kassandra Medrano conducted an unannounced inspection in order to deliver findings on the complaint investigation for the above allegation. LPA Medrano met with the Director, Soni Singh to discuss complaint allegation and finding.

It has been alleged that the facility is not changing children's diapers regularly, leading to the accumulation of waste in the diapers and the development of rashes on the children due to infrequent diaper changes. Based on LPAs observations, record reviews, and interviews which were conducted. The allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

Exit interview conducted and a copy of this report and appeal rights were reviewed and provided to Director, Soni Singh.

Notice of Site visit was observed to be posted and shall remain posted for 30 days.

It was alleged that the facility does not notify parents of all injuries or incidents that occur while in care. LPA Medrano interviewed staff, management, and parents. Based on interviews, observations, as well as information gathered; it was found that facility did not notify parents of injuries sustained in care in a timely manner or until parents addressed the injury with the staff. Even though there are records of the facility reporting injuries to parents. It was found that not all were reported. The allegation noted above is thus found to be SUBSTANTIATED, meaning the allegation is valid because the preponderance of the evidence standard has been met.
