On 4/22/22 at 9:01AM, Licensing Program Analysts (LPAs) Catherine Fernandes, Indira Loza, Melissa Guirit, arrived announced and met Director/owner Renanta Zimany. There were 14 infants in care and seven staff members.

While at the center on a proof of correction inspection LPAs observed the following deficiencies:

- Upon walking in at 9:01AM, infant 1 (I1) was being put to sleep on their stomach while Director Zimany was tapping the infant's back. The infant was sleeping on a mat with a blanket. At 10:43AM the infant still had the blanket over her/him while sleeping.

- While reviewing the roster and checking the sleep log for babies under 12 months, LPA Fernandes observed eight crib age babies enrolled in the center. The license for the center states "a maximum of 5 crib-age infants only." During todays inspection there were seven crib-age infants in care. One of the seven infant's (I1's) mother confirmed that the infant can climb out of the crib and therefore is sleeping on a mat.

Two type A deficiencies are being cited during todays inspection. The Director must provide a copy of this report to all parents of children currently enrolled, and the parents of newly enrolled children in the next 12 months. In addition, form LIC 9224 (Acknowledgment of Receipt of Licensing Reports) must be signed by each parent and placed in each child's file.

A copy of the LIC 9224 is being provided to the Director during the inspection.
