25 | On 10/11/2022, LPA Melanie Otsuji arrived at the facility for an unrelated matter and observed a regulatory violation. Present for this inspection are two staff members and 11 school-aged children in care.
During today's visit at approximately 1:20pm, LPA Otsuji observed a school-aged child standing at the bathroom door frame. LPA looked into the children's bathroom for a staff member. The school aged child (C1) was left unattended. Approximately 30 seconds later a staff member, S1, was observed coming out of a bathroom stall after utilizing the toilet. LPA asked S1 if she was with the child, S1 acknowledged she was supposed to be supervising the child. Both S1 and C1 walked back to classroom together. LPA Otsuji alerted facility Director of the unattended child.
