13 | Licensing Program Analysts (LPA's) Cherie Acosta and Diana Campos conducted an unannounced visit to investigate the above allegation. Present during the investigation was 5 staff and 13 preschoolers in care.
It was alleged that facility was operating beyond the limitations of the license. During the course of the investigation, interviews were conducted, staff files were reviewed. On 4/20/21 LPA's observed a school aged child in care commingled with preschool children. LPA's reviewed school age child's file and confirmed child is in first grade.
Based on the LPA's observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met. Therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division & Chapter Number 101161, are being cited on the attached LIC 9099D.
Exit interview was conducted with Karen Freeman. Appeal rights were provided. Notice of Site Visit was provided and must be posted for 30 days. |
