On July 1, 2024 at approximately 1:40 PM Licensing Program Analyst (LPA) Lorraine Dacanay Breaux arrived a the home and was granted entry by licensee, Alpa Nagar. LPA provided LIC 126. LPA Breaux met with licensee Alpa Nagar for the purpose of conducting an Unannounced 1 year required Annual Inspection. Present in the home for today’s inspection was the licensee, licensee's fingerprint cleared husband, and adult son and 2 preschool age children. The hours of operation will be Monday - Friday, 8:30 AM to 5:30 PM.

This is a two story home, 4 bedrooms and 3 bathrooms. On limits: Family room, living room, kitchen, bathroom on first floor. Off Limits: The entire second floor, which includes all upstairs bedrooms, including master bedroom/bathroom, the bedroom located on the first floor, two car garage, and front yard.

Per the licensee, the ISOLATION AREA will be in the on limits family room away from the other children in care until the parents picl up. Off limits area is made inaccessible by use of gates, closed doors and visual adult supervision. The fireplace is located in the family room and is blocked with glass door. The home has heating and ventilation for safety and comfort. Licensee does not provide meal to children in care and does not provide transportation. LPA reminded licensee to ensure that the safety gate at the bottom of the stairs is secure at all times to prevent access to second floor.

Per licensee, there are no firearm in the home, confirmed there are no pools or bodies of water during today's inspection. Licensee confirmed she resides in the home. All Detergents, cleaning compounds, medications and other items which could pose a danger to children are stored and inaccessible to children. Licensee has age-appropriate toys and learning materials. The home has a fully charged 2A10BC fire extinguisher located in the kitchen near the sliding door. Smoke detector, carbon monoxide detector (tested) and a working telephone. The home has an approved fire clearance for up to 14 children dated 11/4/19. Fire inspector indicates that children are not permitted on the second floor or garage. Licensee is reminded that an assistant is required when operating as a large daycare.

LPA reminded licensee of the following: Mandated Reporter training is to be renewed every two years; CPR/First Aid is also renewed every two years. Baby bouncers & drop-down cribs are not allowed at the day-care facility. LPA asked the licensee does she transports children and the licensee stated that she does not transport children. All required forms are posted and visible for public review.

The OUTDOOR PLAY area is the fully fenced backyard and LPA observed that it is free from defects or dangerous conditions. During today's inspection, there are no play structures which are required to be anchored. There is a BBQ pit that is covered and secured to prevent access to children. There is an old unused water fountain (NO WATER) behind a fence, LPA reminded licensee to have 100% supervision when children are outdoors.

At 2:15 PM, LPA requested and reviewed the files of one (1) children in care and staff. All children’s files contain Identification & Emergency Information, Parent's Rights, and Medical Consent forms and the licensee file. The licensee conducts and documents fire and disaster drills twice a year with the last one conducted on 02/05/2024. The licensee's Health and Safety training is completed, and CPR and First Aid certificate is current and expires on 03/24/2025. The licensee has completed mandated reporter training on 03/08/2024. The licensee is in ratio today.

Effective August 1, 2003, California Law requires Family Child Care Home licensees to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624B). Incidents must be reported within 24 hours by phone, fax, or electronic mail. LPA informed the licensee that all forms can be downloaded at www.ccld.ca.gov and encouraged the licensee to email childcareadvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The licensee was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com.

Per licensee does not administer medication at this time.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

The licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the LICENSEE Alpa Nagar, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

There are no deficiencies cited today. Appeal Rights provided. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee, Alpa Nagar.
