Licensing Program Analyst (LPA) Cherie Acosta and Kayla Merchant conducted an unannounced case management visit. LPAs arrived at the facility to conduct a visit in regards to a complaint. LPAs first met with Shayna Joiner and later with Ann Williams. Licensee was not present during the visit.

When LPAs toured the facility, LPAs observed 7 children present of which 5 were infants. Shortly after LPAs arrival Shayna Joiner left the facility with C7. LPAs were told that C7 was taken to join a field trip at the pumpkin patch with children enrolled in licensee's center, Creative World.

Ennovy Bradford was present during the visit and caring for children. Ms. Bradford does not have criminal record clearance. Ms. Bradford left the facility during the inspection. A civil penalty of $200.00 is assessed today.

The attached type A violation is cited today and must be corrected by the due date. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgement form of proof of receiving this report (LIC9224). The LIC 9224 must be placed in the child's file to be reviewed by licensing.

Report was reviewed with Ann Williams. LPA also reviewed report with licensee over the telephone.
Notice of Site Visit was provided and must be posted for 30 days.

Ennovy Bradford left the facility during the inspection. She may not return to the facility until she has criminal record clearance. Licensee shall submit a letter to CCL by 10/24/24 stating that she understands and shall remain in compliance.

Obtain a California clearance or a criminal record exemption as required by the Department . This requirement was not met as evidenced by: Ennovy Bradford does not have criminal record clearance which is an immediate risk to the health and safety of children in care.

Staffing Ratio and Capacity.For a Large Family Child Care Home, the maximum number of children for whom care may be provided at any one time when there is an assistant provider in the home, including children under age 10 who reside at the licensee's home

Licensee shall ensure that she is in compliance with the required ratio at all times. Licensee shall submit a letter to CCL by 10/30/24 stating that she understands the ratio and shall remain in compliance at all times.

and the assistant provider's children under age 10, shall be either:Twelve children, no more than four of whom may be infants. This requirement was not met as evidenced by during the visit there were 5 infants in care which poses a potential risk to the health and safety of children in care.

Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
