On July 2nd, 2024 at 10:10AM, Licensing Program Managers (LPMs) Wynn Norona and Jason Jang and Licensing Program Analysts (LPAs) Jaleesa Jackson, Manel Estoesta, and Michael Mathew met with Administrator Merat Ayalew at the Oakland Southeast Regional office for an Informal meeting. The purpose of this informal meeting was to discuss the facility operations .

Within the last 12 months, both licenses at this facility has received multiple citations for teacher to child ratio, sign and out procedure, infant food services, and infant safe sleep.

During the meeting, LPM's and LPA's discussed with the Administrator the reasons for the non-compliance with the regulations. The reports dated 4/17/2024, 5/1/2024, and 5/15/2024 were discussed. Technical Support Program as been offered and accepted prior to the meeting today. Facility compliance plan was discussed to provide increase monitoring to provide support.

The Administrator stated that the plan of correction for the lead testing will be sent by the end of the month and the plan of correction for submitting a Director packet to the San Jose Regional office will be sent by the end of this week 7/5/2024. Failure to correct deficiencies by the given due dates, agreed upon during the meeting, could result in a Non-Compliance Conference.

An exit interview was conducted with Administrator Merat Ayalew.

Appeal rights were given and explained to the Administrator Merat Ayalew.
