On October 30, 2024, at 11:00AM, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux conducted an unannounced 1 Year Required Inspection and met with the director, Sheryl Azelton. LPA disclosed the purpose of the inspection and was granted entry into the facility. LPA provided the entrance checklist LIC125. There were 26 children present during this inspection and eleven (11) additional staff members/includes administrative. The hours of operation are 7:30 AM-5:30 PM, Monday -Friday (ages 2 - 6 years)

CLASSROOMS: At 11:20 AM the facility was toured for health and safety, there are two (2) classrooms. Teacher and child ratio was observed, facility is in ratio during today's visit. There are adequate play and learning materials available. The floors, furniture, and equipment are age appropriate and in good repair. The isolation area for sick children is in the director's office, away from the children in care. Drinking water is available inside and outside of the center. There were four (4) fully charged 2 A10BC fire extinguishers, a functioning carbon monoxide detector(tested in staff break room), wired smoke detectors, sprinkler system, first aid kit, and a pull down fire alarm. LPA observed cots for sleeping.

FOOD SERVICE AREAS: This facility provides lunches and snacks for children in care and there are weekly menus posted at the facility. LPA observed an area where they have a refrigerator this area is clean and free of evidence of rodents. Low-fat 1% milk is being served at this facility. Pesticides and other similar toxic substances are not stored in cabinets that contains food. All storage containers for solid waste have tight fitting covers that are in good repair.

OUTDOOR PLAY AREAS: The outdoor play area is fully fenced, there are no bodies of water, or free-standing water accessible to children. Director confirmed there are no pools/bodies of water at the facility. There's a large play structure, that is anchored for safety. This play structure has a slide that has cushion to absorb falls. There are large trees and a canopy that provides shade to children while at play. Families supply water bottles, staff will bring the water bottles outside for hydration and refill as needed. There's a shed in the rear playground that contains equipment, there is a lock to prevent accessible to children.

Records: At 11:35 AM, LPA requested and reviewed children and personnel files. All individuals subject to criminal record review have a clearance and have been associated to the facility. LPA reviewed six (6) children’s files and five (5) staff files. LPA reviewed the facility roster & personnel report and copies were obtained. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Mandated Reporter certificates were reviewed. Director’s CPR/First-Aid Certificate is current and expires on 03/2025. Director was reminded of their responsibility to renew CPR/First Aid and Mandated Reporter certificates every two years. Emergency Drills are recorded and performed at least every six months last one was conducted at 10/28/2024. All required documents are posted in a publicly accessible area.

Criminal Record Clearance: Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Lead Testing: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA referred director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. LPA noted that the Lead testing results was posted for public review.

MyChildCarePlan.org: Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Incidental Medical Services (IMS): This facility does provide Incidental Medical Services (IMS). LPA inspected storage of medications and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Safe Sleep: LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility does not provide care for children under the age of 2 years old.

Unusual Incident/Student Injury Report: Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child’s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days.


No deficiencies nor advisory notes issued during today's inspection.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights Provided. Exit interview conducted and report was reviewed with director, Sheryl Azelton.
