On 2/15/2023 at 8:43am Licensing Program Analyst (LPA) Morgan Pringle met with Director Deidra Daniels, for an Unannounced Annual Inspection. Two (2) classrooms were toured for a health and safety inspection. During LPA’s inspection there were thirteen (13) preschool children and three (3) staff. The facility operates from 7:30am – 6:00pm, Monday – Friday. Facility also has an infant with toddler option license #013422405.

The facility has ample age appropriate materials in the classrooms that were observed to be clean and in good condition. All toxins, cleaning products, and hazardous materials were observed to be in inaccessible areas. All medications are properly documented, stored and maintained. There is at least one (1) fully stocked first-aid kit on site. The two (2) children’s, and one (1) staff bathrooms were clean, free from defects, and in proper working order. All sleeping equipment are clean, free from any defects and properly maintained. All children have access to clean drinking water in and outside of the classrooms. The food preparation area is clean and free from defects. All hazards are properly stored and made inaccessible to the children in care. The facility provides all food for the children. All food provided by the facility is properly stored and labeled. The facility has smoke detectors, carbon monoxide detectors and fire extinguishers on site. LPA did not observe any harmful or unattended bodies of water in or around the facility. The outside area is clean, free from defects with ample age appropriate materials for the children. There is plenty of shade for the children as well.

The facility is operating within their licensed capacity and is in ratio. All required postings are made visible in the entry way of the facility. The fire/disaster drill log is complete with the last drill logged on 12/20/2022. A physical census of the children was taken and cross referenced with the sign-in and out log. Director stated that the staff have been signing the children into the facility rather than their parents and authorized representatives. LPA informed Director that without the children’s authorized representative’s authorization, staff cannot sign children in and out of the facility. During LPA’s inspection it was found that two (2) adults at the facility do not have a criminal record clearance, three (3) staff do not have a Mandated reporter certificate and three (3) staff are missing a health screening report. LPA obtained a sample of the children’s files, facility files and staff files. All children’s files were complete. All staff files were incomplete.

Deficiencies Cited during Inspection
· Two (2) adults caring for children without a criminal record clearance
· Three (3) staff missing Mandated Reporter certificate
· Three (3) staff missing Health Screening Reports with record of negative Tuberculous test

Director was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Personnel Roster must be properly maintained, and fire/disaster drill must be conducted every six (6) months and documented. Director was reminded that California Law requires all facilities to report unusual incidents or injuries to children in care, to child's parents, and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or email. LPA informed Director that all forms can be downloaded at www.ccld.ca.gov. Director was also informed that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com.

LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA Morgan Pringle informed licensee Deidra Daniels that this report dated 2/15/2023 document(s) one (1) Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Pringle informed the licensee to provide a copy of this licensing report dated 2/15/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
