Licensing Program Analysts (LPAs) Jyoti Saini and Kassandra Medrano met with Director Sarah DeSousa for a 10-day complaint visit. In addition to the director, 36 children and 5 staff members are present today. During the inspection, LPAs interviewed the director, staff and received pertinent documents.
Based on the interviews and the self-reported incident, the LPAs determined that the incident occurred during the transition from the playground to the classrooms. Child 1 (C1) and Child 2 (C2) were left unattended on the playground for approximately five to ten minutes. Based on interviews it was found that staff did not conduct name-to-face checks prior to leaving. The preponderance of evidence standard has been met; therefore, the allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101229(a)(1), is cited on the attached LIC 9099D.

Absence of supervision is a Zero Tolerance violation. As a result, a Type A deficiency was issued during today’s inspection. A copy of this report and LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to parents/guardians of current enrolled children in care and all children newly enrolled following a 12-month period of this report. The LIC 9224 must be signed within the next business day the children are in care and is to be kept in the children’s files

An immediate civil penalty of 500$ is assessed today. An exit interview was conducted with director, Sarah DeSousa.



A notice of site visit was given and must remain posted for a period of 30 days.
The Facility's appeal rights were reviewed and given to the director, Sarah DeSousa.

The director shall submit a written plan to the community care licensing division (CCLD) on how the facility and staff will always ensure complete supervision of all children in care. The director shall also conduct in-house training on care and supervision, provide a copy to all staff members, and have them sign the plan.
