Licensing Program Analyst (LPA) Janai McClain met with Director Ayisha Lopes to conduct the complaint investigation for the above allegation. Present during today's visit were 27 children and 3 staff.

During the investigation, LPA conducted facility inspection, observations, interviews, and obtained documents. Interviews and observations indicated that outdoor furniture has sharp corners and jagged edges. Therefore the preponderance of evidence standard has been met, the allegation is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 is being cited on 9099-D page.

LPA informed Interim Director Ayisha Lopes that this report dated 05/14/25 documents one Type A citations, which shall be posted for 30 consecutive days, as there is an immediate risk to the safety of children in
