On June 7, 2024, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux conducted an unannounced 1 Year Required Inspection and met with the Director, Laura Hayes. LPA disclosed the purpose of the inspection and was granted entry into the facility by the director. LIC 125 was provided to director. There were forty-eight (48) children present during this inspection and twelve (12) additional staff members, this includes administrative staff. The facility was toured for a health and safety inspection. The hours of operation are 6:45 AM-5:30 PM, Monday -Friday. This facility has a School Age Day Care Center license #15700526. Staff interview was conducted with director.

CLASSROOMS: There are a total of six (6) classrooms for the preschool. LPA toured the six (6) classrooms and observed the teacher child ratio. There are adequate play and learning materials available. The floors, furniture, and equipment are age appropriate and in good repair. The isolation area for sick children is in the director's office. Drinking water is available inside and outside of the center. There were five (5) fully charged 2A10BC fire extinguisher, a functioning carbon monoxide detector located in the hallway, wired smoke detectors, fully stocked first aid supplies, pull down fire alarm and working telephone. All solid waste storage containers have tight fitting covers on, and appear to be in good repair.

FOOD SERVICE AREAS: This facility provides morning and afternoon snacks for children in care and there are weekly menus posted at the facility. LPA observed an area where they have a refrigerator this area is clean and free of evidence of rodents. Food is protected from contamination and, per staff, contaminated food is discarded immediately. Food/beverages capable of rapid spoiling are properly stored. Low-fat 1% milk is being served at this facility and pesticides and other similar toxic substances are not stored in cabinets that contains food.

OUTDOOR PLAY AREAS: There's a play structure that has wood chips/bark to absorb falls. There's also a large trees that provides shade to children while at play. The staff brings the children's water bottles outside to keep the children hydrated and refill as needed. The playground is fully fenced.

BATHROOMS AND TOILETING AREAS: The staff's bathroom is separate from the children's bathroom. All sinks and faucets are in safe and sanitary operating condition. The children can reach the sinks and toilets. Supplies are available to the children. The bears classroom has a diaper changing table that is in arms reach of the sink.

Lead Testing: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.


RECORDS: At approximately 11:45AM LPA requested children and staff files. All individuals subject to criminal record review have a clearance or exemption and have been associated to the facility. All staff files have required Health Screening, Employee Rights, and Criminal Record Statements and all children files contain Identification & Emergency, Parent Rights and Medical Consent forms. LPA reviewed the facility roster and obtained a copy. LPA reviewed and qualified two (2) teachers. At least one opening/ closing staff member has a current CPR & First Aid Certificate. Mandated Reporter Training was discussed, and certificates were reviewed and completed on 04/23/23. The director's CPR and First Aid certificate is current and expires on 07/24. The center is in compliance with the sign in and out procedure. Disaster drills are being conducted at least once every 6 months and the last one conducted was on 3/24/2024. All required documents are posted in a public accessible area.

HEALTH RELATED SERVICES: IMS is being provided at this facility. LPA inspected the medication, which is stored in a safe place that is inaccessible to children in care. Each of the medications has an unaltered label with the child’s name and date of issuance. A refrigerator shall be used to store any medications that requires refrigeration. The center is equipped with fully stocked first aid kits.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. California Law requires Child Care Centers licensees to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. Roster of the children must be properly maintained, and fire/disaster drill every six months must be documented.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

MyChildCarePlan.org: Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

There's no deficiency cited today. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Exit interview conducted and report was reviewed with the director, Laura Hayes.
