25 | On 07/12/2024 at 10:00am, Licensing Program Analysts (LPAs) Christina Uribe, LPA Morgan Pringle, LPA Michael Mathew, and Regional Manager (RM) Loretta Dyson met with owner Essie Day and site director Dawn Gaddini for a non-compliance conference in the CCLD Child Care Program Oakland Regional Office. Also present during today’s meeting representing CCLD is Licensing Program Manager (LPM) Jason Jang. The purpose of today’s non-compliance conference is to address a recent history of citations issued by the Department to the facility.
During previous visits to the facility LPA Uribe, LPA Pringle, and LPA Mathew have issued 3 Type A violations (immediate risk to the health & safety of children in care) and 9 Type B violations (potential risk to the health & safety of children in care) between 01/24/24 and 06/12/2024. Of these 12 citations, 5 deficiencies were cited during Complaint Visits on 01/24/2024, 04/08/2024, and 06/12/2024 and 4 deficiencies were during Case Management Visits 01/24/2024, 04/08/2024, and 06/12/2024.
The deficiencies stated above were discussed with the Owner and site director. A plan of action was developed and agreed upon by all parties for the facility to be brought into compliance.
LPA Mathew informed the owner and site director to provide a copy of this licensing report dated 07/12/2024 to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
The in-person meeting concluded at around 1:20pm and a virtual meeting was held at 4:00pm with LPA Mathew and RM Dyson to review the report. LPA Mathew reviewed the report with the owner, Essie Day, and site director, Dawn Gaddini. report will be emailed to owner and director for signature.
