25 | On July 31, 2024 at 9:32am Licensing Program Analysts(LPAs) Michael Mathew and Morgan Pringle along with Licensing Program Manager (LPM) Wynn Norona and San Leandro Building inspector Carlos Gutierrez conducted an unannounced case management inspection. The purpose of the inspection is to follow up on the pervious incident when the facility had flooding of sewage water.4 LPAs, LPM and City inspector met with facility director Dawn Gaddini and informed her of the purpose of the inspection. LPAs,LPM and city building Inspector toured the facility inside. There were 27 children, 3 staff members and 2 aids in care at the time of the inspection. During the tour, LPAs and LPM observed children in the toddler room which is located in the back right of the building, and in the gross motor room, located behind the entrance to the building.
At 10:45am the building inspector made a determination to red tag the building for the unsafe sanitary condition. Building inspector posted the red tag on the left side of the main door. The building need to remains closed until the center is cleared by the City of San Leandro. The owner or director will notify Licensing once the City of San Leandro clears the building.
At 1:07pm the facility was placed on inactive status effective tomorrow, 8/1/2024 - 8/30/2024. Owner was instructed she must contact Licensing prior to reopening. An inspection will be conducted to place the facility back to active status.
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
At 1:08pm exit interview was conducted with Director Dawn Gaddini, appeal rights were given, and report was reviewed with the director
