On October 30, 2024, at 8:25 AM., Licensing Program Analyst (LPA) Elimika Woods conducted an announced Pre-licensing Inspection and met with applicant, Ivy Chowdhury. LPA disclosed the purpose of the inspection and was granted entry into the facility by the applicant. Present during the inspection was the applicants fingerprint cleared husband. There were no children present during this inspection. The facility was toured to conduct a health and safety inspection. The facility plans to operate between the hours of 7:00 AM-6:00 PM, Monday -Friday

This two story home consist of four bedrooms, three bathrooms, living and family room, kitchen, backyard, and two car attached garage. The home is neat and clean with centralized heating and ventilation for safety and comfort. The off-limits are will be made inaccessible by closed and/or locked doors and visual supervision. There's a gate at the bottom of the stairs in the living room to prevent children five years and younger access to the upper off limit areas. The Isolation area will be a section of the living room, away from other children in care.

The home has a fully charged 2A10BC fire extinguisher, working smoke detector, working carbon monoxide detector, pull down fire alarm system, and telephone. There's a fireplace in the living room and a fireplace family room that's blocked/screened to prevent by children access.

On-limit-areas are the: Living room, family room, downstairs bedroom, kitchen, downstairs bathroom, and backyard

Off-limit-areas are : Entire second level of home, right side yard, shed, and garage.

The outdoor play area is the fully fenced backyard and LPA observed that it is free from defects or dangerous conditions. There are ample age appropriate toys that appear to be safe and in good condition. There are no pools, hot tubs or any other bodies of water present in the on-limit areas during today's inspection that violates the child care licensing laws. There is a locked shed located in the backyard which is off limits to children in care and a gate with a latch to prevent children from using the right side yard.

LPA did not observe any hazardous materials or toxins accessible to children during today’s inspection. Per applicant, there are no firearms or weapons of any kind in the home. The applicant in compliance with the immunization laws which pertains to all childcare providers. The applicant provided proof of control of property by showing me her Mortgage Statement. The applicant completed and received a certificate in mandated reporter training on 08/04/2023. Applicant's CPR and First Aid certificate is current and expires 07/2026.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-carelicensing/subscribe and select the Child Care option to receive email communication.

Applicant, was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

CPR/First Aid is also renewed every two years. Baby bouncers & drop-down cribs are not allowed at the day-care facility. The applicant was also reminded that Mandated Reporter Training (General and Child Care Providers) is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com

Roster of the children must be properly maintained and fire/disaster drill every six months must be documented. The applicant is reminded any structural changes to the home or additions to the child care facility must be reported to Community Care Licensing.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.

On this date, 10/30/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant, was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Exit interview conducted and report was reviewed with the applicant, Ivy Chowdhury. This home is recommended for Licensure on 10/30/2024.
