On January 16th, 2025 at approximately 9:20am, Licensing Program Analyst (LPA) April Wright met with Licensee Ling Chu Liu for an announced Change of Location / Prelicensing Inspection. Present during today's inspection was the Licensee fingerprint cleared son. The home was toured to conduct a health and safety inspection. Hours of operation will be Monday through Friday from 8:30 to 5:30pm.

The two story home consists of four (4) bedrooms including master bedroom, three (3) bathrooms including master bathroom, Living room (day-care room #1) Family Room (day-care room #2), dining area, kitchen, garage, laundry room and backyard. The home is neat and orderly with heating and ventilation for safety and comfort. There are 2 fireplaces located in day-care rooms #1 and #2. Both fireplaces are not in use and have been blocked with additional child safety locks installed which makes them inaccessible to children in care. There are age appropriate toys and furniture that were observed by the LPA to be in good condition in the day care rooms for the children to utilize. The home has 2 fully charged 2A10BC fire extinguishers and have appropriate smoke/carbon monoxide detectors in place (tested and working). LPA reviewed a rental agreement that shows applicant rents the property.

On Limits: The areas of the day care will consist of Living room (day-care room #1) Family Room (day-care room #2), dining area and gated portion of the backyard.

Off Limits: Entire second level of the home which incudes all three (3) bedrooms and two (2) bathrooms, Kitchen, laundry room, Downstairs bedroom, bathroom, remaining portion of the backyard and garage. Electrical outlets have child protective covers in place making them inaccessible to children in care. Child safety gates and locks are in place in to prevent access to the off limit areas of the home. There is a child safety gate at the base of the stairs to prevent access to the upper level of the home. The gated portion of the backyard play area was observed by LPA to be free from defects or dangerous conditions. LPA observed and Licensee confirmed that there are no pools, hot tubs, or any other bodies of water on the premises. LPA observed and Licensee confirmed that are no weapons or firearms present at the home. All hazardous materials and toxins are out of the reach of children by installing child safety locks on the cabinets and drawers as observed by the LPA. Licensee has three (3) pet cats that are kept in the off limits area of the home. See LIC809C for continuance.

Licensee Preventive Health and Safety training was completed on May 18th, 2021. Licensee CPR/First Aid certificate are current and will expire in March 2025. Licensee completed their Mandated Reporter training on 9/8/2024 and the Licensee is following immunization law as it pertains to all day-care providers.
LPA reminded the licensee of the following: Mandated Reporter training is to be renewed every two years; CPR/First Aid is also renewed every two years. LPA reviewed and discussed with the licensee all licensing forms which pertain to the children’s files and facility files. LPA Wright reviewed and discussed the Safe Sleep practices with Licensee. LPA discussed and reviewed Unusual Incidents Reports with the licensee.

Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Fremont Fire Protection Department granted a fire clearance of this residence on 1/8/2025. The fire clearance was granted with the condition that the garage and the entire second level of the residence will be off limits for day-care purposes. Licensee has a pull down fire alarm that is located in day-care room #1 upon entry to the home on the wall to the right.

Licensee was reminded that all assistants, volunteers, frequent visitors, or adults living in the home, that are 18 years of age or older must be fingerprint cleared and associated to this facility prior to being in the presence of children in care or an immediate civil penalty will be assessed from $100 to $3000 per person, per incident.



Baby bouncers & drop-down cribs are not allowed at the day-care home. The applicant is also reminded that any structural changes to the home or additions to the childcare facility must be reported to Community Care Licensing.

Licensee Ling Chu Liu home is recommended for Change of location on 1/16/2025. Licensee will be issued a new FCCH license that will reflect today's date and new facility license number.

Notice of site visit provided must remain posted for 30 days. This report shall remain on file for 3 years. Report was read and reviewed with licensee Ling Chu Liu.
