25 | On October 30, 2025, at 2:40pm Licensing Program Analyst (LPA) Randy Miranda met with licensee Muqadisa Fazel Rahime for an Announced Case Management Inspection for a Change of Location. Living in the home is the licensee, licensee’s 9-year-old daughter, and 4-year-old-son. Present during the inspection were the licensee, licensee’s son, and zero (0) children in care. The licensee’s home was then toured for a health and safety inspection. The facility plans to operate from Sunday through Saturday, 6am – 10pm.
The facility is a two-story home with three (3) bedrooms, two and a half (2.5) bathrooms, living room, family room, dining area, kitchen, laundry room, backyard, side yards, front yard, and attached two-car-garage.
ON LIMITS AREA: Living room (activity room) dining area, kitchen, family room (activity area), downstairs half-bathroom, and backyard.
OFF LIMITS AREA: Master bedroom #1, master bathroom #1, bedroom #2, bedroom #3, bathroom #2, stairway, and entire 2nd story level. All off limits area will be secured with gates, doorknob protectors, and supervision.
ISOLATION AREA: Living room area. When a child shows signs of illness, he/she will be separated from other children in care.
There were ample age-appropriate toys, all observed to be safe and in good condition. The home was neat and clean, with adequate heating and ventilation to ensure safety and comfort. Safety gates were installed at both entrances of the stairway leading to the second story. A fireplace is in the family room and has been secured with a zip-lock mechanism to prevent access when children are present. Hazardous materials, toxins, and sharp objects were made inaccessible to children.
