13 | On 7/24/25 at 2:45 PM Licensing Program Analyst (LPA) Greg Clark arrived unannounced to deliver findings for the above complaint. LPA met with Anthony Garcia, Administrator and explained the purpose of the visit.
During the course of the investigation LPA interviewed W1, S1 and S2.
S1 and S2 stated that R1 moved into the facility’s memory care unit on 9/10/24 from Kaiser Hospital Oakland’s Emergency Department. Kaiser agreed to pay R1’s first month’s rent and Community Fee. S2 stated that she offered to help W1 find a contact at Kaiser who could authorize payment. To date Kaiser has not paid the outstanding balance. The facility issued a 30-day Notice to Quit to R1 and W1 per company policy.
This agency has investigated the above complain and we have found that the complaint was unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
