25 | An unannounced case management inspection was conducted today at 1:23pm by Licensing Program Analyst (LPA), Erica Laird and Sydney Sims. LPA's met with facility director, Julia Willette in response to an Unusual Incident Report (UIR) received by the Department on 1/17/25. Per the UIR, C1 was playing on a scooter and fell, causing injury to their right arm.
During today's inspection Facility director Julia Willett was interviewed. Julia stated on 1/15/25 C1 was playing on a scooter and fell causing injury to their arm. Julia stated after being medically evaluated it was determined C1 did not break their arm, just sprained it badly.
On 1/28/25 LPA Laird and LPA Sims conducted interviews with two children (C1-C2). Both children interviewed stated C1 had been pushing a tumbling mat on a floor scooter when C1 lost their balance and fell forward. C1 went to catch themselves from falling and landed on their arm, causing injury. Both children stated staff were present when the incident occurred and immediately applied ice to C1's arm.
During today’s inspection, the facility was toured. LPA Laird and LPA Sims observed floor scooters which are used by children in care. LPA Laird and LPA Sims observed C2 demonstrate how the children use the scooters for play.
Based on interviews conducted and observations made, LPA Laird determined the injury did not result from lack of supervision.
There were no deficiencies cited during today’s inspection. Exit interview conducted and report was reviewed with facility representative, Anne Krise. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
