25 | On 2/26/25 @ 10:16am an unannounced case management inspection was conducted by Licensing Program Analyst (LPA), Erica Laird. LPA met with director Linda Wright in response to an Unusual Incident Report received by the Department on 2/19/25. Per the incident report, on 2/17/25 a child (C1) fell from a climbing structure resulting in a broken collar bone.
LPA Laird conducted an interview with director Linda Wright. Linda stated C1 was climbing on the ladder of the play structure and fell. Linda stated the teacher (S1) was standing approximately 5 feet away when C1 fell but did not see if C1's foot slipped or how they fell. Linda stated C1 was assessed for injuries and parents were immediately notified. Linda stated parents took C1 to the emergency room where it was determined C1 had broken their collar bone. Linda stated C1 has not returned to the facility but is expected back.
During today’s inspection LPA Laired inspected the climbing structure and took 2 photos. LPA Laird noted the highest wrung of the ladder is approximately 2-3 feet off the ground. The playground has adequate cushion and the play structure is age appropriate.
There were no deficiencies cited during today’s inspection. Exit interview conducted and report was reviewed with director, Linda Wright. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
