32 | LPA Mendez interviewed parent (P1-P2) regarding child having an unexplained injury while in care. P1 stated that they were informed that their child (C1) had sustained an injury on their face and spoke with licensee regarding the incident. P1 had received a text and a photo of C1’s injury. P2 stated child (C1) had a bruise on their cheek, when C1 was picked up and were notified about C1’s incident and had no concerns.
LPA Mendez interviewed two staff (S1-S2) on 3/7/22 and 3/10/22 and addressed the allegation of child that received an unexplained injury while in care. S1 stated that the child (C1) had tripped over their own two feet and hit their face on a table and wrote an ouch report and talked to parent. S2 stated that had witnessed child fall, S2 stated that C1 had tripped and fell and hit their face against the table. S1 stated that they had grabbed C1 and placed an ice pack on C1’s face and wrote an ouch report and provided one copy for the parent (P1) and one copy for C1’s file.
LPA Mendez interviewed director Lauren Thompson on 3/11/22, regarding facility not reporting communicable disease. Director Lauren Thompson stated that they sent out notices regarding foot and mouth outbreak and possibly pink eye outbreak.
LPA Mendez interviewed eight staff (S1-S8) on 3/7/22, 3/8/22, 3/10/22, 3/11/22, 3/24/22 3/29/22 and 4/12/22. LPA Mendez addressed with staff facility not reporting communicable disease, LPA Mendez asked staff (S1-S8) if parents were informed of any outbreaks and 6 out of 8 stated yes, parents were informed verbally and by letter.
LPA Mendez interviewed 20 parents on 3/2/22, 3/7/22 3/14/22, 3/15/22, 3/17/22, 3/24/22,3/28/22 and 3/29/22. LPA Mendez interviewed parents and asked if facility was communicating with families in regards to communicable disease and if they were notified. 11 out of 20 parents were notified by letter or by text message regarding communicable disease outbreaks.
During today’s visit facility was toured and observed 28 children present.
Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated. An exit interview was conducted. The Notice of Site Visit must be posted for 30 days. |
