25 | Application Specialist (AS) Alecia Sifuentes and Licensing Program Analyst (LPA) Tiffanie Diep met with Applicant, Sarah Kelly for the purpose of an announced pre-licensing inspection. Applicant requests a school-age license to serve 28 school-age children from kindergarten and above. The program will operate Monday through Friday from 7:00 a.m. to 6:00 p.m. The facility will be providing afternoon snack during the school year and breakfast, lunch, and snacks during holidays and summer.
LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
INDOOR ACTIVITY SPACE:
There is one school-age classroom/multipurpose room. AS observed a sufficient amount of equipment, tables, and chairs. The first aid kit and medication will be located in the sick room. AS observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. AS observed a water pitcher in the classroom. AS observed a functional carbon monoxide detector in the classroom. AS observed a sign-in/sign-out binder.
AS measured one classroom. The total classroom space contains a total of 580 square feet, which will only accommodate 16 school-age children. There is 1 toilet and 1 sink for the children so AS informed applicant that they can only accommodate 15 school-age children. There is a separate private restroom for the staff. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). Children who become ill during the day will be isolated in the sick room and will use the staff restroom, if necessary.
