A Case Management Visit was conducted today 8/26/21 by Licensing Program Analysts (LPAs) Melissa Domantay and Melissa Guirit. LPAs met with Director, Linda Frazier-Stafford and on boarding site supervisor Director Michele Freeman. Facility is a Title V program. An application was received for a toddler option and decrease of capacity. The preschool program will operate in Classroom 1 and 2, and the toddler option will operate in classroom 4. The preschool and toddler option program will operate from 6:45AM - 5:45PM, Monday through Friday. A health and safety inspection was conducted inside and outside and the measurements are as follows:

PRESCHOOL:
INDOORS: 1584.71 square feet = 45 children
OUTDOORS: 6894.13 square feet = 92 children

TODDLER OPTION:
INDOORS: 805. 46 square feet = 23 children
OUTDOORS: 4032.35 square feet = 53 children

The preschool and toddler option rooms are equipped with varied age appropriate materials and equipment. There are 3 toilets, 3 sinks available for preschool children and 3 toilets, 3 sinks for toddlers. The play therapy room and the staff toilet in the play therapy room will serve as isolation areas for sick children. There is a kitchen available. Mats are available. The 2 play yards are fenced in all around. The small climbing structures are sufficiently cushioned with poured rubber and turf. Canopies provide sufficient shade in the play yard. The center has functioning drinking fountains inside and outside, as well as jugs and cups. The center will provide am/pm snacks, breakfast, and lunch. No cleaning agents were found stored with food. First aid supplies are available in the center. There is a working telephone on site. Carbon monoxide is available. Facility will utilize electronic sign in/out.

This facility plans to provide Incidental Medical Services – IMS. A Plan of Operation that includes IMS must be submitted to the Department. Linda Frazier-Stafford is a fully-qualified preschool director and has completed 16 hours of health and safety training including pediatric CPR and First Aid and Lead Poisoning Training.

Mandated reporter, appeal rights, COVID 19 protocols, Provider Information Notices (PINs), unusual incident reporting and fingerprint requirements were discussed today. Licensee is also being informed of the web address (www.ccld.ca.gov) for downloading child care forms, and the director is encouraged to email ChildCareAdvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The director is also reminded that mandated reporter training is required for all staff and is to be renewed every 2 years at www.mandatedreporterca.com.

Zero Tolerance policies were explained. The center was found to be clean, safe, sanitary, and in good repair.

A license for a toddler option for 23 children in 1 classroom #4 and a license for 45 preschool children in classroom #1 and #2 will be issued effective 8/26/21 .

An exit interview was conducted with Director, Linda Frazier-Stafford. A copy of this report was provided via email due to printer issues, notice of site visit, and appeal rights was provided to Director Linda.
