On Friday, May 5, 2023, 2:40 PM, Licensing Program Analyst (LPA) Caroline Colson met with Marilyn Doss, Center Director and Tana Reed, Site Administrator l, for an unannounced case management inspection. There are 45 preschool children and 24 staff members including the director. Documentation was reviewed. An incident occurred when a teacher tried to stop a child from running around in the classroom. Another teacher intervened and grabbed the child and placed the same child on the floor. The same child was held down by the teacher with her hand on his back.

The attached type A deficiency is being cited today and must be corrected by the due date. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. All parents/guardians must sign an acknowledgement form of proof of receiving this report (LIC 9224). The LIC 9224 must be placed in the child's file to be reviewed by licensing.

Notice of site visit was posted at the time of the inspection and must be posted for 30 days. An exit interview was conducted. Appeal rights were given and discussed. This report must be available for public review for 3 years.

Licensee removed the teacher from the classroom. The deficiency was cleared during the inspection.

A teacher grabbed a child by his clothes and placed the child on the floor. The same child was held down by the teacher with her hand on his back. This requirement was not met as evidenced by document review and licensee interview. This poses an immediate health and safety risk to the children in care.
