On 11/7/24 at 8:30 Licensing Program Analysts (LPA) Mario Caro and (LPA) Christina Watts conducted an unannounced Subsequent Complaint Investigation at Amirkhani, Armineh. LPAs met with Licensee and explained the purpose of today’s inspection. Present during the visit were licensee, 1 fingerprint cleared staff, 4 infants and 1 preschooler. Licensee stated there are 8 chldren enrolled. The finding for the above allegation was delivered during the inspection. During the course of the investigation LPAs completed a physical plant inspection, and conducted interviews. Interviews stated facility has not been out of ratio. Based off observtaion of todays visit and the initial complaint visit the facility is not out of ratio. Based on the interviews and information obtained throughout the investigation, the allegation is UNSUBSTANTIATED which means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. No Deficiency has been cited for this allegation. Exit interview conducted with Licensee. Appeal rights were provided.

A NOTICE OF SITE VISIT WAS ISSUED AND MUST BE POSTED FOR 30 CONSECTIVE DAYS.

LPAs met with Licensee and explained the purpose of today’s inspection. Present during the visit were licensee, 1 fingerprint cleared staff, 4 infants and 1 preschooler. Licensee stated there are 8 chldren enrolled. The finding for the above allegation was delivered during the inspection. During the course of the investigation LPAs completed a physical plant inspection, and conducted interviews. Interviews stated parents have been in the facility on many occassions and have not been denied access. It can not be determined at this time that parents have been denied access to the facility. Based on the interviews and information obtained throughout the investigation, the allegation is UNSUBSTANTIATED which means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. No Deficiency has been cited for this allegation. Exit interview conducted with Licensee. Appeal rights were provided.

LPAs met with Licensee and explained the purpose of today’s inspection. Present during the visit were licensee, 1 fingerprint cleared staff, 4 infants and 1 preschooler. LPAs Can not determine at this time whether or not staff are or have been force feeding children. Per LPAs observation during todays visit the children are eating their food. Based on the interviews and information obtained throughout the investigation, the allegation is UNSUBSTANTIATED which means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. No Deficiency has been cited for this allegation. Exit interview conducted with Licensee. Appeal rights were provided.
