25 | On 10/11/2024 at 12:00 PM, Licensing Program Analyst (LPA) Christina Watts conducted a Case Management Inspection at Baby Yale Academy. LPA met with Owner, Lita Reeves and explained the purpose of this visit. During today's visit, there were 6 school age children in care with 1 staff in one classroom. Owner stated there were 25 school age children enrolled. All staff caring and supervision children have Criminal Record Clearance.
LPA conducted an a visit on an unrelated incident, LPA became aware of an incident occurred in the facility. LPA inspected the facility, reviewed records and conducted interviews. Multiple interviews stated C1 was on top of C5's shoulder and C1 fell off C5's shoulder. C1 sustained a broken arm 2 two places. Multiple interviews stated S1 did not witness the incident and school age children had to inform S1 of the incident. California Code of Regulation, Title 22 states staff are required to visually supervise children at all all times. This incident is in violation of California Code of Regulation, Title 22.
LPA Christina Watts informed Owner, Lita Reeves that this report dated 10/11/2024 with 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Furthermore, LPA Watts informed the Owner to provide a copy of this licensing report dated 10/11/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with the Owner, Lita Reeves. A Notice of Site Visit was given and must remain posted for 30 consecutive days.
