25 | On 10/17/2024 at 10:45AM Licensing Program Analysts (LPA) Kareeca "Reeca" Sykes and Cherie Acosta conducted an UNANNOUNCED CASE MANAGEMENT- License initiated visit to add the backyard as an ON-LIMIT area. LPA's met with licensee Nicole Cannada to discuss the purpose of today's visit and was granted access into the facility to conduct a tour for health and safety. During the visit there was one (1) infant child present.
During the inspection LPA's inspected all ON-LIMIT areas of the home including the backyard
The ON-LIMIT AREAS: The complete downstairs (excluding the garage and laundry room)
The OFF - LIMITS AREAS: The complete second floor of the home, the garage, and laundry room
During the tour LPA's inspected the backyard. There is a play structure that is anchored to the ground and is secured. LPA's observed the play structure to have splinters. LPA's inspected the area while observing the backyard LPA's observed a portion of the right side of the yard to be off limits and made inaccessible by gates. The left side yard is off-limits to children and made inaccessible by gates.
Prior to having the backyard approved as an ON-LIMIT area licensee will submit the following corrections:
- Licensee agrees to have the the play structure sanded and free of splinters.
- Licensee has four (4) BBQ pits and propane tanks. Licensee shall make these inaccessible to children and submit proof.
Licensee agrees to have all corrections made by 10/24/2024.
Exit interview conducted, appeal rights were given, and report was reviewed with the Licensee Nicole Cannada. Notice of site visit was provided to licensee and must be posted for 30 days. |
