1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 6/4/2025 at 9:00am Licensing Program Analyst (LPA) Morgan Pringle met with Director Amy Lundberg for an Unannounced Required - 3 Year Inspection. Present during the inspection were forty-nine (49) preschool age children, six (6) additional staff members, one (1) volunteer and the director/owner of their second school, Kayla Young. Five (5) classrooms, Room 1 (Green), Room 2 (Orange), Room 3 (Yellow), Room 4 (Blue), and Room 5 (Red) were toured for a health and safety inspection. The facility operates 7:00am – 6:00pm, Monday – Friday. LPA observed all classrooms to have ample age-appropriate materials for the children’s learning and play that were clean, properly stored and in proper working order. All toxins, cleaning products, and hazardous materials were observed to be in inaccessible areas. All medications on site were observed to be properly documented, stored and easily accessible to staff for emergencies. LPA observed all bathrooms to be clean, well maintained, with ample number of sinks and toilets. The diaper changing station in room two (2) was observed to be properly padded, clean, and free from defects. All diapering materials were properly stored as well. All napping materials were observed to be well maintained, properly stored and labeled. The facility provides sleeping cots and mats, and all bedding is brought from the children’s home. There are multiple properly working smoke detectors, carbon monoxide detectors, fire extinguishers, and first aid kits on site. The facility does not transport children. Page 1 of 4

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:

Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA observed the kitchen to be clean, well maintained, free from defects and all hazards have been made inaccessible to the children in care. All food on site was observed to be properly stored. The facility gives the option for children to bring their lunch from home or receive a lunch provided by the school. All food that is brought from the children’s home was observed to be properly labeled and stored. All counters were observed to be clean and well maintained. All children bring their own water bottles and have access to clean drinking water in and outside of the classrooms.

The outside play yards were observed to be fully fenced, well maintained, with ample age-appropriate materials for the children. All play materials for the children were observed to be clean and in good repair. The play structures were observed to be anchored into the ground with ample cushion for children’s safety. There are locked storage sheds that are inaccessible to the children in care. There is plenty of shade for the children. LPA did not observe any bodies of water in or around the facility.

The facility is operating within its licensed capacity and is in ratio. All staff working at the facility have obtained a criminal record clearance or have transferred their clearances. The fire/disaster drill log is complete with the last drill logged 4/16/2025. A physical census of the children was taken and cross referenced with the electronic sign-in and out log. All children were properly signed in by their parent or authorized representative. LPA obtained a sample of the children’s files, staff files, and the facility files. All files were complete.

No deficiencies were cited during LPAs inspection.

Director was reminded that California law requires facilities to report unusual incidents and/or injuries to children in care, to the child's parents, and to the Department within 24 hours by phone. Within seven (7) days from the incident, facilities must submit the Unusual Incident/Injury form (LIC 624) to the Department. Director was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years.

Mandated Reporter Training ("Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting https://mandatedreporterca.com/. Personnel and facility roster must be properly maintained, and fire/disaster drills must be conducted at least every six (6) months and documented. LPA informed Director that all forms can be downloaded at www.ccld.ca.gov.

LPA referred Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY) and link to publication. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
