On 10/09/2025 at 1:00PM Licensing Program Analyst (LPA) Kareeca "Reeca" Sykes conducted an Unannounced Subsequent Complaint Investigation at Crystal Gatti's Large Family Childcare Home. LPA met with Licensee Crystal Gatti and explained purpose of today's visit. LPA's observed three (3) preschool age children and one (1) school age child in care, and licensee states there are five (5) children enrolled.
Complainant alleges that an "injury wasn't reported to the parent". During the course of the investigation, LPA's inspected the facility, reviewed records and conducted interviews.
Based on the evidence obtained and interviews that were conducted throughout the investigation, the licensee did not report the injury to the parent. P1 contacted the licensee to inquire about what happened to C1's leg while in care. The licensee stated that "she scratched it". It was later discovered that C1 had possibly sustained the scratch while playing or siting on the bricks in the backyard area of the facility. Therefore, the allegation is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 is being cited on 9099-D page. Exit interview was conducted with Licensee Crystal Gatti. Appeal rights were provided.
A NOTICE OF SITE VISIT WAS ISSUED AND MUST BE POSTED FOR 30 CONSECUTIVE DAYS.

As soon as possible but no later then the same business day, the licensee shall notify a child's parent or authorized representative regardless of the injuries or acts that affect that child as specified in Health and Safety Code Section 1597.467(a). This requirement has not been met as evidenced by:

Licensee stated that by close of business 10/16/2025, the licensee will write a statement and submit it to licensing on how they understand that they must report all injuries that happen to children to the parents/guardians of the children and remain in compliance.

Based on evidence recieved, that the licensee failed to report an injury to a parent. It was later determined C1 may have injured their leg playing on the bricks in the backyard. Which poses a potential risk to the Health, Safety or Personal Rights to children in care.

Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until coorrected.
