25 | On February 18, 2025, Licensing Program Analyst (LPA) Soleil Marx met with Director, Melissa Schmierer, for the purpose of conducting a case-management inspection regarding an unusual incident report. The purpose of today's inspection was explained.
It was self- reported that on February 6, 2025, an infant needed medical treatment after sustaining nursemaids elbow while in care. It was learned through interview and written record, that a staff member tried to help an infant back down to the ground who had climbed and was standing on a low table top. The staff member lifted the infant down from the table by holding one of the infants hands, which resulted in a dislocated elbow joint. Although there did not appear to be bad intent from the staff in question, not utilizing proper handling techniques resulted in injury.
The facility notified LPA Marx by written unusual incident report on 02/12/2025. Based on 101212 Reporting requirements, the incident needed to be reported to LPA Marx or Sac RO by the next business day, which put the facility out of compliance with reporting requirements.
One type A and one type B citation are being issued on the 809-D page.
Director was informed that this report dated 02/18/2025 documents one Type A citation which shall be posted for 30 consecutive days. The Licensee shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report reviewed with Director. Appeal rights were provided. A notice of Site Visit was given and must remain posted for 30 days. |
