25 | On March 13, 2025, Licensing Program Analyst (LPA) Soleil Marx, met with Facility Representative, Jennifer Tommasini, for the purpose of conducting an unannounced case management inspection to follow up on a self reported incident. The purpose of today's inspection was explained.
During today's inspection, LPA observed a census of 25 infants being supervised by four staff, in three separate classrooms during nap.
It was reported to Sacramento Regional Office that on March 7, 2025, the facility called emergency personnel for an infant that choked on tanbark while inside the classroom. Staff were prompt in noticing the signs of the infant in distress and quick to intervene. Staff administered back trusts and were able to partially dislodge the tanbark to open the airway. Emergency personnel arrived to the facility and medical treatment was needed. The infant was released from the hospital with no significant injuries from the incident.
During today's inspection, LPA made observations of facility environment and protocols, reviewed records relevant to the incident, and interviewed staff that were present during the incident.
LPA verified the facility is in compliance with CCR 101212, reporting requirements.
The facility has implemented policies and procedures as preventative measures to minimize tanbark and other small debris from being tracked into the infant classroom.
The facility has increased cleaning of the floors throughout the day, is installing netting over tanbark in infant outdoor activity space, has eliminated through traffic of the classroom, and will place a LifeVac anti-choking device in each classroom in case of emergencies.
No Title 22 deficiencies were cited.
Exit interview was conducted and a copy of this report was given to the Facility Representative. A Notice of Site was provided and must remain posted for 30 days. Appeal rights provided.
